As a Diabetes Educator, I provided patient-centered education and support to individuals living with diabetes, helping them understand their condition and empowering them to manage it effectively. My role involved conducting individualized assessments, creating tailored care plans, and teaching patients essential self-management skills such as blood glucose monitoring, proper nutrition, medication adherence, and lifestyle modification.

I collaborated closely with physicians, nurses, and dietitians to ensure holistic and coordinated care, while also monitoring patient progress and adjusting interventions as needed. I educated patients on recognizing symptoms of hypo- and hyperglycemia, proper insulin administration, and strategies to prevent long-term complications. In addition, I facilitated group sessions and counseling to enhance patient confidence, motivation, and long-term adherence.
